No Game: The Injured Reserve 4.5
Crop Dusters only dressed 3 defensemen for some reason. Left 1.5 on the bench in the form of Braydon Coburn. (Had 3 other D-men who are injured/suspended... no effect on final result)
Nope, has played the past 6 games, dating back to Nov 8th.
And I've said it over a hundred times. People should under no circumstances be leaving lineup spaces open when they have players on the bench, even if they're injured. You never know when a guy will be activated just before the game and play. There's literally NO DOWNSIDE AND NO RISK to making a guess and gambling on a player if you have a bunch of injured guys and an open spot.
I generally set my lineups a week in advance and then go back and edit as necessary on a daily basis. That way if I completely forget, I know I have at least some form of a lineup set.
I left that spot blank with the intention of going back and figuring out who was likely to play as the day got closer, but I had an early holiday party last night and completely forgot to check back in.

No worries. I've just taken to pointing out lineup irregularities for everyone, just to keep us all on our toes, diligently adjusting our lineups. :)
That seems to be the approach most people take. Set lineups a month or two at a time, then fine tune as the game day nears. That's certainly my approach.
